Our registration process is straight forward, simply call us and we can arrange for a referral form to be emailed to your veterinary surgeon.
If you prefer, you can download the form yourself and arrange for your Veterinary surgeon to complete it prior to your appointment.

The Veterinary consent form must be completed and present at your first appointment, without it we can not commence with any treatment. This is because we work from veterinary referrals only. The Veterinary Surgeons act states that only a qualified Veterinary surgeon can determine if your pet is physically fit enough for treatment. It also allows your hydrotherapist to work as part of an MDT (Multi disciplinary team) which gives you and your dog a wider support network and a more accurate treatment programme to achieve optimum results. Simply click the icon ‘Veterinary Consent Form’ and download the form and pass to your vet to complete.
The new Data Protection Regulation comes in to effect on the 25th May 2018.
As a valued customer of Willow’s we have your details on file.
The details include your name, address, email and your telephone number. These details will not be shared with third party companies. We will however, keep your referring Veterinary Surgeon updated on progress, where your details will be shared.
We do not store payment details of any kind.
Our commitment is to provide you a highly professional, caring and friendly service. We kindly request you to read the terms outlined below and sign your agreement to them on the Willow’s hydrotherapy veterinary referral form/client registration form.
All animal’s treatment at Willow’s require a signed and dated Veterinary referral form. This can be brought with you on the day of your first appointment. Alternatively you can post the form to 65 Mowbray Drive, Blackpool, FY3 7UN or email it to info@willowshydro.co.uk
Payment for services is due on the day of your booked appointment. Receipts are provided as requested by yourself. Payment methods include cash or card payments.
We agree that occasionally you may need to cancel your booked appointment and our cancellation policy requires a 24-hour notice period. For cancelled appointments outside of this period or non-attendance, the appointment fee will be charged in full. Willow’s will make all efforts to contact owners if an appointment needs to be changed or cancelled, but will not accept liability for any losses or damage.
If your animal has an infectious or contagious condition you are required to inform us and reschedule your appointment; the cancellation policy applies. If your bitch comes into season, you must inform us as she will not be able to attend any booked appointments until her season is over.
It is essential you do not feed your dog 3 hours before your booked appointment. Specialist advice for protocols will be given for you to follow post appointment.
Please ensure your dog has had plenty of time to toilet prior to the session. Please clear up after your dog in the Willow’s grounds. If the pool is fouled, an additional £50.00 will be charged due to the extra cleaning required and pool closure.
Willow’s do have small parking facilities available but we will not be held responsible for any damage, theft or injury caused by persons using the car park.
Please keep your dog on a collar/harness and lead at all times in the car park and clinic. Although your dog may be well behaved and friendly, there could be other animals at the clinic that are overwhelmed or hurt by normal friendly dog behavior.
Please ensure you wear clean, sensible footwear when attending appointments. Wet floors are unavoidable in the clinic, especially in the wet room. We understand that at certain times of year it can be a challenge to keep your animal mud free. We kindly ask that you bring your animal as clean as possible to the booked appointment.
Please ensure you bring plenty of towels to your booked appointment to dry off your dog and help keep them warm post session. In the older months a dog coat is essential.
Our water management is maintained to the highest of standards at all times. Documentation is kept for 2 years and can be shown on request.
